The photo from the OP looks good. I just looked at my kaoko instructions. The plastic sleeve goes under the throttle part, then the knobby wheel, then the bark buster end.
Disregard my other stupid post above. I'm going to just fill my handlebars with lead shot. I might as well since I'll have the bar ends apart.
