if it were a mfg error, then why is it in an assembly? Seems it would have been tossed upon the faulty forming, rather than bolted up to a rod cap and bearing cage.

I know nothing of the manufacturing techniques for engine components of this scale. Maybe that's the jig used to hold the rod for forming?
