if it were a mfg error, then why is it in an assembly? Seems it would have been tossed upon the faulty forming, rather than bolted up to a rod cap and bearing cage.

I know nothing of the manufacturing techniques for engine components of this scale. Maybe that's the jig used to hold the rod for forming?

Look at the broken rod bolt. That was a running failure, not a mfg error. I have a rod and piston out of my sprint car engine that looks sort of like like that (the rod bolt).
