Zoom in on the rear hub. Lots of tandems have a large finned brake drum on the back that you can set to drag from a lever on the handlebar . On long downhills you set that to keep you from running away like a loaded semi truck. Trying to use only the rim brakes will heat the rims up enough to blow tires. It was bad enough with two people on board. No thanks to four.

I don't think you need to worry about going over the bars with a cantilever brake on a bike that long with that much weight so far out on the lever arm. Melting the pads - yes. Popping the tire - definiitely. Stopping - maybe. I'd have some big discs on it with a hell for stout front fork.

That is an Arai drum brake. It is usually set up as a drag brake with a friction shift lever set up to control it. It has as much brake shoe as many sixties motorcycles.
