The closest thing available now is the Spot 2 or Spot Connect.

Packing a GPS receiver, satellite phone and power supply into a watch-sized package isn't easy or cheap - the sat phone antenna alone is rather large. A simple cell-based unit doesn't fit the criteria. It won't work in quite large areas of North America, nevermind worldwide!

Yeah, because the tech they get is both affordable and available to consumers

Another option might be the Interphone GPS Safe.

In Spanish however (I believe): http://www.youtube.com/watch?v=pv5pvOsZvNE

there is an app on the iphone that allows for something like this. app is called motionx-GPS. unfortunately, you will need to have some sort of data signal for this to work properly.

What about a personal epirb? Not quite watch sized but will work anywhere and could in theory* get a better response than Spot.

*not getting into THAT debate

A PLB won't allow friends and family tracking though.

True. However the only time he mentioned tracking was after he sent the SOS, rather than the Spot like follow me type feature. Redundancy is good, carry both

A combination of an EPIRB or PLB (for emergencies) and smartphone (for telling family where you are) is ideal I think.

Got a Spot device and used it on my last ride, does everything you mention and I'm very pleased with it.
